Graduation Preparation Forms

University Core Audit

As a graduating senior, you need to download the University Core Audit form and fill it out. All forms must be completed, signed (if necessary) and turned in by the deadline to room AD104, the B-wing of the Administration Building.

Department Audit

As a graduting senior, you will need to print and fill out the Department Audit for your major. It must be signed by the department chair and your academic advisor. If you have more than one major, forms for each major must be completed and signed.

All forms must be completed, signed and turned in by the deadline to room AD104, the B-wing of the Administration Building.

Minor/Concentration Audit

If you have a minor or interdisciplinary concentration you need to download the Graduation Audit of Minors and Concentrations form and fill it out.

It must be signed by the department chair for minors and director of the concentration for interdisciplinary concentrations. If you have more than one minor or concentration, forms for each must be completed and signed.

Graduation Application

Each student in the College of Arts and Sceiences must fill out a Graduation Application in order to recieve a diploma. The Graduation Application is available online here.
