Forms and Petitions
Students who have obtained sophomore status are permitted to take up to six courses on the Pass/Fail basis with restrictions outlined in the Undergraduate Bulletin under the Academic Policies and Procedures section. Please refer to those guidelines. Print the Pass/Fail form, complete the form, and make an appointment with the Assistant Deans Office to have the request approved.

Courses in which a C-, D+, or D grade is earned may, with the appropriate dean’s permission, be repeated only once, and the permanent record then includes the grades of both attempts. For the purpose of computing academic standing, both attempts are counted, but credit toward graduation is given only once.
To request permission to repeat a course, please complete an Academic Petition.
For students who demonstrate superior ability, an instructor may suggest a plan of independent study that shall include some remission of the obligation of regular class attendance. Independent-study plans in each case shall have the recommendation of the department chair and approval of the appropriate dean.
Please print the Independent Study Contract and complete all areas including securing the appropriate signatures. After final approval by the dean, it is the student’s responsibility to register for the course with the Registration Office in the Student Service Center.

Please read the following steps for completing and processing the Transient Petition form:
- Your last 30 credit hours must be taken at John Carroll University. Only credit hours, NOT grades, will transfer to John Carroll University.
- Complete the student information section and the university information section. The total number of hours you have completed must be filled in.
- Fill in the course number, course title, and credit hours at the host institution.
- The John Carroll University department chair will fill in the JCU equivalent. You will need the signature of the JCU department chair.
- Indicate how you want the class to transfer to JCU. For example, core requirement such as a letter designation or division, major, minor, or concentration. A course description is required for core course.
- Once you submit the electronic transient petition, you will recieve a confirmation email with additional information for tracking the process of the petition.

In the event that a student requests a substitution of courses, a special request of the Core Committee, an extension of time to complete course work, or to request the Freshmen Privilege option, an Academic Petition is used.
