RELEASE OF INFORMATION

Authorization for Release of Information

A properly completed "Authorization to Release Information" form is required in order to release personal information in either of the following circumstances:

• In order to have information released to the University Counseling Center from another provider or,
• In order to have the University Counseling Center release information to another provider
