ASSESSMENT REQUEST/APPROVAL FORM

In order to ensure effective assessment projects in the Division and to avoid duplication of efforts, all assessment projects and instruments must be approved by the Assistant Vice President for Assessment and Planning in Student Affairs. No assessment may be started without this approval. Please submit the following information via this form.
