PROFESSIONAL DEVELOPMENT REQUEST FORM

Requests must be reviewed and approved by the supervisor to whom you ultimately report (Dean of Students, Senior Director of Athletics and Recreation or Assistant Vice President for Student Development and Assessment) prior to being submitted on-line to the Vice President for Student Affairs for final approval. Staff members may only begin the registration process after they have received approval from the Vice President.
