JCU Account Action Form

New Accounts at JCU are created automatically when new employees are entered into the main system by Human Resources. Likewise, student accounts are automatically created when students are accepted into the University. This ITS form should only be used to request creation, or deletion of accounts that are not automatically created or deleted by regular procedures. If the person already has a banner ID but no account, than you should use this form. This form can also be used to modify existing accounts or to request special accounts that are not included in the normal procedures described above.

If you have any questions about the status of an employee, please contact the HR Department before proceeding.
