Your IRB application may be returned before it is reviewed if it
is difficult to read or understand, or if it is incomplete. Handwritten
applications are discouraged.
Reviewers often requested additional information or minor changes
to material or procedures. Listed below are some of the more frequent
reasons for revisions.

not in the same field. |

Information is missing on how the consent form and other
material will be collected. |

Detail is lacking on how the data, including the consent
form, will be protected
and who will have access. |

Consent form/protocol does not differentiate between confidentiality
and anonymity. |

Typographical errors are found on consent form and/or survey
