A properly formatted Birth Date List must be submitted within 5 business days of the event, and must include ALL of the following:

  1. An indication at the top of the spreadsheet detailing organization name, location of event, and date of event.
  2. A list of names for all attending members AND their dates.
  3. List of birth dates for all persons attending the event.
  4. A Yes/No column next to each listed birth date for identification of those of legal drinking age (Ex: “21 or Older).
  5. All attending members over the legal drinking age will have their names clearly highlighted on the spreadsheet.
  6. A count at the bottom of the spreadsheet indicating the total number of attending persons of legal drinking age.
